Bringing Improved Quality of Life
Each patient starts with comprehensive geriatric assessment to determine the medical, psychological, and functional capabilities and limitations. A treatment plan is then developed and shared with the patient’s primary physician. S+AGE geriatricians continue to serve as geriatric specialists, available as needed, to assist the primary physician with long-term care or specific medical issues.
S+AGE recognizes that each elderly patient is a unique individual. Our staff is friendly and knows each patient by name. Initial evaluations take about two hours; follow-up meetings are scheduled as needed.
Physician to Physician
Geriatric disorders are common in physician practices and can be time intensive and difficult to manage. Geriatric patients and their caregivers often need to be engaged in active learning about diagnostic and treatment options. S+AGE provides this through specialized diagnosis and management. The multi-disciplinary team at S+AGE includes:
- Board Certified Physicians with specialty in Geriatrics
- Geriatric Nurse Practitioner
- Registered Nurse
- Licensed Vocational Nurse
- Social Workers
- Psychologist
S+AGE is not a primary care practice. Physicians receive a detailed report of evaluation results with specific treatment recommendations and periodic communications with S+AGE geriatricians regarding patient status and treatment.
Patient Follow-up & Referral to Resources
Patients receive a comprehensive evaluation of physical, cognitive, emotional, and functional status and written care instructions for the patient and the caregiver.
Referrals are made to community resources for services and support, in addition, a synopsis of medical and medication history is provided for use by other physicians or in the event of an emergency.
Most of the services at S+AGE are covered by Medicare as well as many insurance plans.
Free Memory Screenings
Early detection of memory loss is important in preventing further decline. Memory Screening is an effective way to detect memory impairment. S+AGE offers free memory screening by appointment. Call 818.341.SAGE (7243) to schedule an appointment.
